When a patient receives a transplant, they are by definition immunocompromised and the possessor of a rare and expensive commodity . They have an obligation to do what they can to preserve that transplant to the best of their ability. This includes behaving in a way that minimizes the risks associated with immunocompromise as well as avoiding behaviors that will put the organ at risk.
It is common for candidates for transplantation to have a number of requirements they must fulfill. These include (for various types of transplants) such things as agreeing to no longer use alcohol, tobacco, recreational drugs, as well as getting all recommended vaccines.
There are far more patients needing organs than there are organs available. If you are in charge of choosing which patient will get an organ, who will you choose?
